Single Person Annual Membership
With membership: unlimited home visits (within reason), no co-pays for visits, direct access to Dr. Jatoi via cellphone, text, email, telemedicine platform 24 hours a day and 7 days a week. This includes prescriptions to be sent electronically, called in, or written on prescription pad.
Married Couple/Partner Annual Membership
With membership: unlimited home visits (within reason), no co-pays for visits, direct access to Dr. Jatoi via cellphone, text, email, telemedicine platform 24 hours a day and 7 days a week for two members who are married or domestic partners. This includes prescriptions to be sent electronically, called in, or written on prescription pad.
Single Home Visit Non-Member Rate
Only scheduled in advance on a case-by-case basis, cash price visit to review your medical conditions, reconcile home medications, write/eRx prescriptions for non-controlled medications for six months, and review any current medical concerns.
Initial Registration Fee
Initial registration into the practice, including documents, gathering of previous records, creation of EMR record, setup in the recurring payment system for monthly fee
Re-enrollment Fee
Free to cancel your membership with thirty days notice, if you decide to re-enroll at a later time
